Thrive Family Practice is happy to see patients with endometriosis and/or persistent pelvic pain. Patients can self refer, or can be referred in by their GP.
Thrive is thrilled to see patients who have an existing relationship with their own GP.
Information for GPs
If you'd like to send your patient along for a review with us, please send us a referral through the usual channels - see our contact us page. We especially like communicating via HealthLink; our EDI is thrivefp.
Where patients have an existing GP relationship:
- All patients will be seen initially for a one hour assessment with one of our GPs
- We will write to you (their GP) with a summary of that visit
- We usually suggest endo patients have a care plan completed. This is preferably done by you as their GP; we can provide suggestions of who to include in that patient's team
- We would love to include you as the patient's usual GP in one of our monthly multi-D case conferences
- Your patient will be invited to participate in one of our Endo First Aid groups
- We will aim to see the patient as required for endo/pelvic pain matters over a 12 month period and then hope to hand them back to you at the end of this time
